Angus Council and the Angus Health and Social Care Partnership have reviewed their current Framework for the provision of personal care, support and care and housing support (including home-based respite services) under SDS Option 3 within the homes of supported people in Angus. This is a procurement exercise to establish a new framework, commencing 01 April 2027. This is an opportunity for providers in the Care at Home sector to tender for various Lots within the new framework. The new Framework will run for four years with options to extend for a further three years and then a further two years. The estimated values in this notice are based on the Framework running for a total of nine years

Contract for Integrated Community Lifestyle Services, procured via the The Health Care Services (Provider Selection Regime) Regulations 2023. The aim of the contract is to improve population health outcomes and reduce health inequalities through the delivery of accessible, integrated prevention services within community settings. The service shall seek to: - Improve access to preventative healthcare; - Reduce smoking prevalence; - Increase engagement amongst underserved populations; - Support sustainable behaviour change; - Improve cardiovascular health outcomes; - Contribute to reducing health inequalities across Peterborough. The Provider shall: - Deliver NHS Health Checks within community settings; - Increase uptake of NHS Health Checks amongst eligible residents; - Deliver evidence-based Stop Smoking support; - Improve smoking quit outcomes; - Prioritise routine and manual workers; - Prioritise residents living within social housing; - Prioritise residents living within private rented accommodation; - Deliver evening and weekend appointments; - Provide workplace-based outreach clinics; - Procure and manage Smoke Free App licences; - Procure and distribute vaping products in accordance with national guidance; - Deliver pilot nutrition and healthy lifestyle interventions; - Support residents to achieve sustainable behaviour change; - Reduce health inequalities through targeted delivery to underserved populations. The contract term is 24 months, anticipated to commence 26th October 2026 and expire 25th October 2028

Healthy Child Programme The objective of the service is to deliver the healthy child programme to all babies, children and young people aged 0 to 19, or up to the age of 25 for care leavers and those with special educational needs and disabilities (SEND). The age criteria cover 0 years to 18 years and 364 days - up to a child's 19th birthday or 24 years and 364 days up to a child's 25th birthday. This is the national delivery model by which public health nursing services are provided. The aims of the healthy child programme are to: • ensure a healthy start to life • identify and address health needs early • prevent health problems • support school readiness and learning • protect children and families • promote health equity Tendered under PSR competitive process. For the avoidance of doubt PA23 and PCR2015 do not apply.
